After Home Health Services ends, if outpatient follow-up is warranted, the therapist will make the appropriate referral. The outpatient scheduler will call the family with a specific time and place. 

Depending on your family member’s functional status, if traditional outpatient services are not appropriate, then there are different options throughout the community that offer activity programs designed to maintain a healthy lifestyle.
